Sickness
- Self-Certification
of Sickness and Return to Work Form
(submit by email)
- For absences up to and including seven calendar days. This form requires Adobe Acrobat Reader. If you have a version of Reader older than 7.0.5, you may be unable to use some of the automated features of the form. You should still be able to complete it and attach it to an email manually. Anyone with an ISS account can access version 9 of Reader via RAS (under 'accessories').

Absence Record Of Discussion (SARD) Form
- This form must be completed by the Supervisor/Manager following any informal
return to work discussion with employees regarding their sickness absence
for absences of eight days or more, the Stage 1 Informal Attendance Review
and for all stages of the formal review process.

- This form must be completed by the Supervisor/Manager following any informal

- This form must be completed by the Supervisor/Manager following every formal review of a disabled employee’s attendance review/case conference.

- Sickness reports must be returned by 5th of each month direct to Payroll
