Completing the Research Passport Occupational Health Questionnaire
Once you have received your Research Passport Occupational Health Questionnare you will need to : -
- Complete the Occupational Health (OH) form
and where relevant
- Obtain records of vaccinations given (e.g. a print out of GP vaccination records)
and
- Place the OH questionnaire/ vaccination records in a sealed envelope with your name and "OH Questionnaire" marked clearly on the front. Put this envelope in another envelope and post directly to the Medical Sciences HR department. This will enable HR to track the status of your application and the unopened document will be forwarded to OH.

- OH will then undertake any additional screening/immunisation status checks as deemed necessary.
- Subject to the nature of work to be undertaken and in compliance with the relevant NHS Trust Infection Control policy, OH will forward staff for MRSA screening . If this is required a copy of the OH screening questionnaire and fit slip confirming health clearance level will be forwarded to the relevant NHS OH Service for action. Thereafter the employee will be contacted by the NHS OH Service for screening.

Following the pre engagement health checks OH will issue a fit slip certificate to the University HR office confirming current health status and health clearance level. HR will then sign off the Research passport form and forward this to the member of staff together with the fit slip. The researcher must then submit this documentation to the relevant R&D research passport office who will issue either the Honorary Research Contract or letter of access.
Note: - Staff will be contacted by their OH department (i.e. where they hold a substantive contract) every 3 years to check that their health status has not changed and issue an updated fit-slip. This does not need to be face-to-face contact and can be done via telephone or email. If there is a change in role, and a higher level of clearance is required the substantive employer should undertake additional checks and issue a new fit slip.
