
A randomised controlled trial of colorectal polyp and cancer prevention in carriers of Hereditary Non-Polyposis Colon Cancer (HNPCC)

CAPP is a programme of genetically targeted clinical trials. The acronym originally referred to the European funding programme Concerted Action for Polyp Prevention. The new meaning for the acronym recognises the involvement of 37 centres in 16 countries worldwide.
The primary objective of CAPP2 is to determine, by randomised controlled trial, whether daily ingestion of aspirin and/or resistant starch will reduce adenoma initiation and progression in this genetically predisposed population.
Participants will be randomised in a factorial design to 600mg aspirin and/or 30g "resistant" corn starch with placebo controls. The primary endpoint will be the incidence of bowel neoplasia. A secondary endpoint will be the incidence of associated extracolonic malignancies. The number, size and histological status of all colonic adenomas and carcinomas will be recorded and compared with the treatment and placebo groups. The study will recruit 1000 people from centres across the UK and abroad. To be eligible for CAPP2 participants must be either a proven carrier of a pathological mutation in one of the mismatch repair genes or for clinical diagnosis, belong to a recognised HNPCC family based on the modified Amsterdam criteria and have been treated for an HNPCC related cancer. Recruitment began in March 1999 and we hope to complete by December 2004.
