Referral Procedure
The Occupational Health Service can deal with a wide range of work related health issues. Two critical areas of referal are:
For all employees referred to occupational health a medical record will be established and, on first attendance, employees will be asked to complete a registration form.
If you are uncertain about the reason for your referral to occupational health you are strongly advised to discuss the matter with your manager and, if necessary, your Human Resources Officer prior to attending for your appointment.
During your appointment you will be issued with a Consent Form which explains the referral process to you - this must be signed by you before a report to Human Resources can be released.
