How can health services contribute most effectively to facilitating successful transition of young people with complex health needs from childhood to adulthood?

Transition is an important, confusing process where young people move from child to adult services. For example, young people who need regular medication and supervision may have poor health and social outcomes if they drop out of services. Disabled young people are often not prepared for the different services they face at age 18. Young people with autism often have mental health problems that are not managed well during transition. The Programme will investigate how Transition services in the NHS can be improved:
